About St. Luke's Clinic – Heart Failure: Meridian

St. Luke’s Heart Failure Clinic is the only clinic in the region with cardiologists who are board certified in advanced heart failure and transplant cardiology. Our expert team partners with you and your family to create a personalized care plan focused on education, symptom management, and improving your quality of life. We aim to help you better understand your condition, reduce hospital readmissions, and slow disease progression.

Your care may include support from a registered dietitian for heart-healthy nutrition guidance, as well as coordination with other specialties such as home health, palliative care, and hospice services. For patients needing advanced therapies, we collaborate closely with Intermountain Medical Center in Utah to provide access to heart transplant and left ventricular assist device (LVAD) surgeries.

To support your journey, our financial advocate is also available to help you navigate health-related costs and coverage.
