It’s not hospice nor is it end-of-life care, but rather specialized support focused on improving quality of life at any stage of a serious health condition. Our team of experts works closely with you and your family to provide care that's tailored to your unique needs, offering guidance and comfort throughout your journey.
This type of care often supports individuals living with chronic or complex conditions such as cancer, heart disease, cystic fibrosis, dementia, end-stage liver disease, kidney failure, lung disease, and stroke. Our goal is to ease the challenges that come with these illnesses by helping manage symptoms like pain, nausea, difficulty breathing, loss of appetite, sleep issues, fatigue, and emotional or spiritual distress.
We're here when you need us:
- In the hospital: your attending provider can refer you or you can ask your medical team if this is a good option for you.
- Outside the hospital: talk with your healthcare provider about a referral, or call us to learn more about our services.
