MyChart Proxy Access Instructions
MyChart is a useful tool that helps patients manage their own health care and it can also help caregivers support the care of another person. When someone accesses the MyChart account of another patient they are doing so as their “proxy,” and this kind of access is called “proxy access.”
Users obtain MyChart proxy access (a) by demonstrating that they have a legal right to access the patient’s information, or (b) when an adult patient grants them such access. For more information regarding MyChart, please visit our Terms of Use.
Setting Up Proxy Access
In order to set up proxy access for a child's MyChart account, you must first have your own MyChart account. If you need a MyChart account, please sign up online. If you need assistance, call (208) 381-9000.
To set up proxy access to your child’s MyChart account, please submit the request directly from your MyChart account.
- Access the Menu > Proxy Request Forms
- Review the information regarding proxy access
- Submit the “Minor” proxy request form.
Please allow one business day to complete the request. Your proxy access to your child’s MyChart account will appear in "My Family's Records" the next time you log in to MyChart.
